The Developmental Disabilities Health Home (DD Health Home) is intended for individuals statewide served through the Division of Developmental Disabilities (DD) who have a qualifying chronic health condition or are at higher risk of health issues or destabilization. The DD Health Home provides population health management and assists individuals with meeting their health and wellness goals while ensuring they get the care they need.
