Structured Family Caregiving Waiver Required Daily Documentation Reminder
Structured Family Caregiving Waiver (SFCW) caregivers must adequately document on a daily basis the services provided to participants and submit this documentation to the provider agency to support that the services were rendered. Providers must submit this documentation to the State upon request. Failure to have documentation may result in recouped claims.
Daily documentation to support services rendered includes caregiver notes that record and track the following:

Daily documentation to support services rendered includes caregiver notes that record and track the participant’s status, updates or significant changes in their health status or behaviors, participation in community-based activities, medication management records and other notable or reportable events.
