SFCW Required Daily Documentation Reminder
Structured Family Caregiving Waiver (SFCW) caregivers must adequately document on a daily basis the services provided to participants and submit this documentation to the provider agency to prove services were rendered. Daily documentation to support services rendered includes caregiver notes that record and track the participant’s status, updates or significant changes in their health status or behaviors, participation in community-based activities, medication management records and other notable or reportable events.
An electronic visit verification (EVV) system may be used if the EVV records include the required caregiver notes; however, the SFCW does not require the use of EVV.
Providers must submit this documentation to the State upon request. Failure to have documentation may result in recouped claims.
