The MO HealthNet Division (MHD) Provider Forms webpage has been updated to include the following revised Department of Health and Senior Services (DHSS) regional office maps:
• Healthy Children & Youth/Medically Fragile Adult Waiver Regional Map
• Department of Health and Senior Services/Division of Senior & Disability Services - Regional Managers Map
These maps offer regional office contact information for participants and providers that have participant referrals or questions about DHSS home and community-based services (HCBS) offered in their area.
