The Missouri Perinatal Quality Collaborative (PQC), funded by the Missouri Department of Health and Senior Services and the Missouri Hospital Association, and in collaboration with The Uplift Connection powered by Missouri Foundation for Health, needs your help in collecting information around perinatal Substance Use Disorder (SUD) resources across the State of Missouri. In this effort, PQC has collaborated on a survey collection tool for those providing perinatal SUD services. The results of this survey will help update the currently existing public-facing resource map for providers and mothers alike and will take approximately 20 minutes to complete.

This survey will ultimately improve care coordination and promote improved health outcomes for mothers, infants and families across the state. Only certain information will be made publicly available—all other information is for planning purposes of the PQC to inform and shape future projects that will improve the lives of Missouri families. By completing this survey, providers indicate approval to share the following information publicly:

  • Provider name, location, phone, and website
  • Services offered
  • Insurance/payment type accepted

This short survey should be completed by someone in a lead supervisory role with a strong knowledge of programming and operations.
