Providers who believe documentation regarding services for Missouri Medicaid participants was destroyed or lost as a result of a natural or man-made disaster should complete the Attestation of Medical Record Loss or Destruction form found on the Missouri Medicaid Audit and Compliance (MMAC) website within 30 days of the date the event occurred.
