Expand Your Referral Base for Participants with Prediabetes and Offer Participants Diabetes Education
Diabetes Prevention Program
Diabetes Prevention Program (DPP) services for eligible adult participants are covered through the MO HealthNet Program. DPP is a structured, lifestyle change program specifically developed and recognized by the Centers for Disease Control (CDC) to prevent or delay Type 2 diabetes. The program is intended for people who have prediabetes or are at risk for Type 2 diabetes, but who do not already have diabetes.
DPP services include ongoing nutrition or behavioral counseling focusing on weight reduction and lifestyle changes and physical activity and fitness assessments.
Studies show that completing the program reduced participants' chances of developing type 2 diabetes by 58% compared to placebo (71% for individuals aged 60 and older). This was nearly twice the reduction seen in the group taking metformin (31%).
DPP services require a referral or prescription in the participant’s plan of care from a physician or other licensed practitioner.

Diabetes Self-Management Training
MHD also offers Diabetes Self-Management Training (DSMT) to eligible adult participants.
DSMT refers to services used in the management and treatment of type 1, type 2, and gestational diabetes. These services can be prescribed by a physician or health care professional with prescribing authority to a Certified Diabetes Educator (CDE), Registered Dietitian (RD), or Registered Pharmacist (RPh). Participant visits include an initial assessment and two subsequent visits, in an individual or group setting. Sessions are tailored to individual member needs.
